If all of your supplied information is correct and validated, we will send the card to your postal address that we have on record within three (3) weeks.
Only Mastercard or Visa accepted. For any other payment method please contact SafeWork NSW on 13 10 50.
All personal details must exactly match SafeWork NSW's records.

NOTE: If you have changed address since receiving your last card and not notified SafeWork NSW then:

Do not proceed with this application.
Update your address details by completing an Update Contact Details request.
Or, complete the Replacement Application Form to change address and request a replacement licence.
